You may sign into the patient portal at the top of the page. Do not pre-register. You do not need to make a new account. Your account is your first name, last name, and year of birth. Example: JaneDoe1984. If you have not received a temporary password, then contact the office. You will be sent an email with a temporary password. Once you are in the portal and have agreed to the consent to use the portal, you may update your personal history, ask for refills, request appointments, and look at you billing account.
You may request an appointment by pre-registering. We will contact you within 1 business day to schedule your new patient appointment. After you have been confirmed, we will send you an email with a user name and temporary password. Please login and fill out your family history, social history, medical history and review of systems.
